Before your first visit
Patient forms
Your consultation form is to be completed and brought to your first appointment. It helps Kylie assess your health condition and any contraindications before commencing treatment.
Please also bring any doctor’s letters, prior assessments, garments, bandages and products you are using for your condition. A doctor’s authorisation is required for previous cancer patients within a five-year period of clearance — the treatment authorisation form above is the letter for your doctor to sign off. Patient discount
Bodyscience compression
All patients receive a Bodyscience discount when undergoing treatment with Kylie. The Bodyscience leggings have been proven to give a certain amount of compression for when you are not wearing your garment and when reduction has taken place.

You need to register yourself. Visit the Bodyscience site and, under ‘login’, use the ‘register’ button. Enter the friend’s code above when you register to receive the discount.
The discount does not include already-discounted items or packs.
Booking and confirmations
Appointments are booked in advance and paid in full at the time of booking. Treatments are very much in demand, so if you need to change or cancel, please do so as early as you can — it lets Kylie offer the appointment to someone on the waiting list.
