Before your first visit
Patient forms
Your consultation form is completed online — for a first appointment it is part of booking, filled in and signed before payment. It helps Kylie assess your health condition and any contraindications before commencing treatment.
Please also bring any doctor’s letters, prior assessments, garments, bandages and products you are using for your condition. A doctor’s authorisation is required for previous cancer patients within a five-year period of clearance — the treatment authorisation form above is the letter for your doctor to sign off. Booking and confirmations
Appointments are booked in advance and paid in full at the time of booking. Treatments are very much in demand, so if you need to change or cancel, please do so as early as you can — it lets Kylie offer the appointment to someone on the waiting list.
