Your consultation form is completed online. If this is your first appointment, it is part of booking — you fill it in before you pay. It gives Kylie the background she needs to assess your condition and any contraindications before commencing treatment.

Your details, case history and any conditions Kylie needs to know about before treating you. It also covers consent for Low Level Laser therapy, and includes a dedicated section for lymphoedema and lipoedema patients.
There is nothing to print and nothing to sign on paper. Booking your first appointment walks you through it — it takes about five minutes, and you sign it on screen before payment. Your confirmation email then carries a private link, so you can come back and update your answers any time.

Where a doctor’s approval is needed
Treatment authorisation form
Some conditions call for your doctor’s written approval before treatment — including previous cancer patients within a five-year period of clearance. This letter asks your GP or specialist to authorise Manual Lymphatic Drainage and the methods Kylie may use.
Take it to your doctor, or forward it to their practice — they can respond to Kylie directly by email.
