Please download, complete and bring the consultation form to your first appointment. It gives Kylie the background she needs to assess your condition and any contraindications before commencing treatment.

Your details, case history and any conditions Kylie needs to know about before treating you. It also covers consent for Low Level Laser therapy, and includes a dedicated section for lymphoedema and lipoedema patients.
Print it, fill in the client sections, and bring it with you — the clinical pages at the back are for Kylie to complete during your assessment.

Treatment authorisation form
Some conditions call for your doctor’s written approval before treatment — including previous cancer patients within a five-year period of clearance. This letter asks your GP or specialist to authorise Manual Lymphatic Drainage and the methods Kylie may use.
Take it to your doctor, or forward it to their practice — they can respond to Kylie directly by email.
